The Better Health Care Connections: Models for Short Term, More Intensive Health Care for Aged Care Recipients Program will provide $25 million (including indexation) over five years to support initiatives to encourage aged care providers to work with public and private health care providers and medical insurers to deliver short term, more intensive health care services.
Better Health Care Connections: Aged Care Multidisciplinary Care Coordination and Advisory Service
The Better Health Care Connections: Aged Care Multidisciplinary Care Coordination and Advisory Service Program will provide $9.969 million over five years to support older Australians with complex health needs who would benefit from increased access to multidisciplinary teams of health professional to coordinate care and treatment. The Program also includes an innovative pilot of GP consultations via video conferencing to Residential Aged Care Facilities.

eHealth records and older Australians
Retirement can be a busy time of life. You want to stay active and maintain the best possible health. The government’s new personally controlled electronic health (eHealth) record system will help you take control of your important health information.
An eHealth record is a secure online summary of your key health information. You can use it to track things like your current medications, allergies and recent medical treatments. And having this information available to the people involved in your healthcare – your doctors and nurses – will help them make the best possible decisions about your health needs.

Aged Care Complaints Scheme - HACC complaints item
The Aged Care Complaints Scheme (the Scheme) will be able to examine complaints about Commonwealth funded HACC services from 1 July 2012. This is part of a broader change, where the Commonwealth will directly fund HACC services for older people in all states and territories, except Western Australia and Victoria. This covers people aged 65 and over and Aboriginal and Torres Strait Islander people aged 50 and over.
People will continue to be encouraged to speak to their HACC service provider first to resolve any concerns unless a care recipient’s health or safety is at risk. Visit the Scheme’s website for more details about HACC complaints.

Aged Care Complaints Scheme: complaint resolution toolkit now available
The Aged Care Complaints Scheme’s new complaint resolution toolkit is now available to help aged care industry effectively handle complaints and to support the introduction of the new framework for complaints resolution.
The toolkit aims to help providers understand their roles, responsibilities and involvement in complaints handling, the components of an effective internal complaint resolution process, and the Scheme’s role and new complaint resolution framework. You can access the toolkit online at: http://agedcarecomplaints.govspace.gov.au/toolkit.
